In this project, we created an in-patient medicinal education and symptom-recognition tool for patients with COPD (Chronic Obstructive Pulmonary Disorder) to transition more smoothly from hospital to home. The discussion guide empowers the patient to recognize and manage post-discharge symptoms and take the necessary steps to maintain a healthy lifestyle.
“I learned about the potency a communication tool to break systemic organizational silos and to unify the efforts of many medical professionals towards improving the delivery of care. I also learned the importance of understanding the tone of voice and the framing of the message. In a focus group reviewing our deliverables, we received both negative and positive reviews based on how we shuffled three words around on the document. This exercise helped us make seemingly small edits which ultimately had a highly positive influence on the receiving audience.”
